Important Information – Free GP Visit Card for patients Aged 70 and Over

We would like to let you know that all patients aged 70 and over are entitled to to a free GP visit card regardless of income.

This card allows you to visit your GP free of charge, helping to make your healthcare more accessible and affordable.

If you have not yet applied for your GP visit card, you can do so easily online at www.hse.ie/gpvisitcardover70 or you can complete the paper application form attached.

Please return the completed forms to the address on the form. Your card will be issued by the HSE and is valid for all routine GP visits once approved.
